I swear a past episode showed people running to or from the right side (as viewed from inside Cheers) of the door, but the exterior shots we finally see of the stairs leading down show a wall there. Both possibilities—my memory being flawed, and the producers making a continuity mistake—are pretty equally possible here, honestly.
IMDB says the title is a reference to Star Trek: The Motion Picture (1979), and it probably is. I imagined it could be a subtle reference to that scene in Spaceballs (1987) wherein the characters discuss merchandising, but it's unlikely: when "Cheers: The Motion Picture" aired, that film wouldn't come out for almost three more months.
