Eiji Hino is a traveling man who has no place to call home and a tragic past. When metallic creatures known as the Greeed awaken after their 800-year slumber to attack humans and feed off of their desires, the disembodied arm of the Greeed named Ankh gives Eiji a belt and three Medals to fight the other Greeed as Kamen Rider OOO. The mysterious Kougami Foundation approaches Eiji and begins assisting him in his fight against the Greeed, though their true motives are not clear. As Eiji fights the Greeed and their Yummy monsters, learning more of the Greeed and Ankh, he starts to find a purpose beyond his journey.

A director's cut version of the last two episodes of Kamen Rider OOO put together. It was released to DVD and Blu-ray on February 21, 2012.

Being revived alongside Ancient OOO, Kazari appears and attacks Hina Izumi and Chiyoko Shiraishi. When all the hope seems lost, Eiji Hino returns and transforms into Kamen Rider OOO, successfully defeating Kazari for the time being. After that Eiji and Hina spend time together eating ice cream and reminiscing about their time together with Ankh.

The year is 2021 and the world is in a state of chaos and fear. The ancient OOO has come back to life after 800 years of sleep. Shintaro Goto and Akira Date, who once fought the Greeed, fight back as the resistance. Hina Izumi, Chiyoko Shiraishi and others support them. And Eiji Hino, who had gone on a trip, returned. Then, something happens to that "broken core medal", and Ankh is revived. What will be the fate of Eiji and Ankh?

The story depicted in "The Secret Birth of Birth X" is the "fantastic debut" of Kamen Rider Birth X, which appeared in the V-Cinext "Kamen Rider OOO 10th: The Core Medals of Resurrection". Hino Eiji, Izumi Hina, Date Akira, Ankh, Greed and others run amok in a parallel world. Another world of "Kamen Rider OOO" that can never be seen in the TV series or V-Cinext is a must-see!
