There was no human error here! I blame the rules, the rules were broken and they were followed to the letter, which sadly led to a disaster. So I blame the ICAO first, because there were multiple close calls before and they never acted, they wanted to see blood before they change the rules. And SkyGuide for their reckless and obviously dangerous rules of letting one controller handle everything.
