The cameras return to Swansea's streets to follow the desperate and moving stories of the homeless. We try and find out what has changed in the city which boasts the highest numbers of homeless people in Wales. Long-term homeless Tim is still sleeping out in one of the harshest winters and continues to have constant run-ins with the police. Geroghika the Romanian who was sleeping in someone's loft has now become a local celebrity and 32-year-old Scott reveals how to build a shelter out of wheelie bins.

Some of Swansea's most desperate homeless people face freezing conditions, urgently trying to find somewhere to live. Susan fell out with her family and has no choice but to sofa surf with friends. But she gets turfed out onto the streets and has nowhere else to go. Scott, from Llanelli, was evicted from a homeless hostel and is now sleeping rough. This time he's sleeping in an abandoned car. He's close to freezing to death whilst his mates have a bed for the night at a nearby church shelter. Paul, the one and only happy long-term Swansea homeless person, originally filmed for last year's series, continues sleeping on the streets, but he's back on the road and we now discover him living on Canterbury's streets.

In the final part of this revealing series, we meet Andy and his 72-year-old dad Cookie, both surviving on Swansea's streets throughout the coldest winter weather for over a decade. Both are trapped by their alcoholism and both are constantly moved on by the local police. Andy is so desperate to escape the streets he turns to the church to help him get into a rehab programme. Two Christmases ago we filmed Stevie on the streets, he was also struggling with addiction and was even thinking about breaking the law just to get a prison bed. Now a year later, we meet him again, his life has been transformed. We also follow the unique and desperate stories of those seeking emergency shelter when their lives are on the edge.
