I figured the twist out during the opening scene but I kept watching to see how they pulled it off.

First, unless I'm mistaken, male/female twins can only be fraternal, which means they form from different eggs. The doctor comments on the egg no fully splitting so Gabriel should have been a female or was a trans character. I do hope he's not intended to be trans because that would make this whole thing very transphobic since he's the insane parasite that kills everyone while the pretty perfect CIShet white girl is just the innocent victim.
Updated (because I'm a nerd and love to share things I learn): Apparently there are two very rare instances that identical twins might have different genders. One is called semi-identical twins in which two sperm fertilize a single egg. Normally this can't happen because the first sperm stops any others from entering, but if they both arrive at once it might be possible. The second is when an XY egg splits and one of the resulting embryos is missing the Y chromosome for some reason. The first seems more plausible with this scenario since the second occurs during the full split. However, both of these cases are as rare as the whole parasitic twin thing so I would think that it should have been included in the reveal. It could have been used to explain why she had enhanced strength and speed as him even though they shared the one body.
Second, Madison would have been scarred from the top of the back of her head down to her butt crack based on the videos. No one even mentioned that though. How does a family take in a child with that type of scarring and not take it into account at all?
It you overlook the major questions and holes, the film was pretty standard fare for this director. Lots of mediocre acting. Lots of screaming. Lots of poor lighting and people who don't turn on lights. Lots of volume cranking to try and make it scarier. Lots of head scratching when it's over. The concept was interesting but the actual movie was a mess.
Oh, and that ending, WTF? She should have offed herself because she will never see the world outside a maximum security prison or mental asylum again.
