The story follows a brain surgeon named Minakata Jin who has spent the last two years in anguish, as his fiancee lies in a vegetative state after an operation he performed. One day, he faints at the hospital and awakens to find himself transported back in time to the Edo Period. He is soon attacked by a samurai, but he escapes with the help of a man named Kyotaro. Kyotaro suffers a serious injury to the head while trying to protect him, but Jin manages to save his life despite a lack of proper medical equipment. Because of that, Kyotaro's sister Saki begins taking an interest in Jin and becomes his assistant. Meanwhile, Jin is determined to find a way back to the present.

A twenty-first century Japanese physician finds himself in mid-nineteenth century Japan and soon becomes involved in court intrigue and medical emergencies requiring his futuristic skills.

Having noticed a slight change in the photo of his fiancé and him, Jin wonders if saving Ryoma has altered the future. He decides to try to stay out of events in the village to prevent anymore changes. However, as cholera sweeps the village, the medicine men call upon Jin to give them the cure. He tells them that he does not know. As hard as he tries to stay neutral, Jin's decision is challenged when Kiichi, the son of the woman he saved, becomes ill. Saki confronts him, as he asks her to return to her home. However, his fondness for the boy, changes his mind.

Now that Jin has Koan Ogata, the supervisor of the Western Medicinal Office for the village, and his team on his side, Jin begins to create a large-scale plan. He asks if rubber hoses exist and if a syringe needle can be made to create an IV drip. The town begins to make saline solution, sterilize everything with alcohol, and burying waste and burnt clothes in large ditches. Ryoma takes it upon himself to be the leader of waste disposal. It is now winter of 1862. Jin had confessed to Saki that he is from the future. However, she has gone about as if nothing has happened. Kiichi is an apprentice at Daikichi's shop. Ryoma has become Katsu's student, while Kyotaro is his secretary. Jin is lecturing at the Western Medical office, with Saki in attendance and Ogata as his most fervent and out-spoken student. One day, as Jin is creating a tool that could drill a hole in a patient's skull, he and Saki get too close. She jumps away and asks him what he wants for dinner. She agrees to make him a special dish.

After the check-up with Oyoji, Courtesan Nokaze asks Jin to see another patient. He discovers a former courtesan in the last stages of syphilis. There is no cure, since penicillin has not yet been discovered. Jon tries to persuade the rest of the prostitutes at the brothel to accept to an examination for signs of syphilis. However, they call him a pervert and refuse. Feeling defeated for his inability to help Courtesan Nokaze, Jin suddenly recalls the first meeting with Miki.
