I liked the episode from the point of the idea for the story. But, again, the self contained episodic nature left a lot of questions unanswered. That, and the fact that some of the delusional episodes where way too cheesy. But it can serve as character back-up.
One thing, though. The Captain ordered Kes to the bridge and she acknowledged that. But she stays in sickbay where we see her next. Is there a missing scene or did she just disobey. Of course, having her on the bridge could have prevented the illusions there which would have made a difference. Guess the writer had a bid of a hick-up.

The miniature doctor is adorable.

Janeway's holo novel is actually quite interesting. The more we learn from this story, the more mysterious it is that private Janeway indulges in such a story. Does she like to play the servant cause she wants a pause from the burden of command?

It's maybe not the greatest episode (at least the stakes are high) but I like it for telling us more about Janeway. Usually Star Trek captain's get all the intention but Janeway was strangely neglected thus far. (And it's nice to see B'Elanna [rather inconsequentially] shagging and to see Tuvok's weirdly attractive wife).
